Immediate actions refresh at the beginning of your turn, don't they? Or is it the end, and my group just plays funny?

If they refresh at the beginning, there's your action for Third Eye Clarity.

Immediate actions effectively refresh at the end of your turn since using an Immediate action prevents the character from using their Swift action for the next turn.

Alas, Jack is correct. 

Besides, in regards to Third Eye Clarity, it's kind of a moot point; since activating it is indeed an action, which you can't do because you're dazed (so for that specific purpose, it doesn't really matter when they refresh).

Just a note:
I got a little lazy when it came to filling in the specific options for immediate actions (e.g., items, maneuvers, etc.).  I'm sure I'll get to it eventually; but I'll probably miss anything that is setting-specific.

(formatting aside) where can I improve this?

Anticipatory Strike is basically psionic Greater Celerity, but it doesn't cause you to be stunned next round. It does basically rob you of your full round action next round, but if you use it well, you shouldn't need that anyway, right? :D

Should synchronicity (CPs, lvl 1 power) be mentioned as well? It's not an immediate action, but it is an interruption (in most cases).

You won't find many for class features, but:
Absorb Spell (Spt20) Redirect absorbed spell or cast any spellthief spell as an immediate action.

The mark of stars feat (Dragonmarked, p142) prevents you from being flat-footed no matter what.  Unfortunately, it requires taking a level in heir of Siberys (Eberron Campaign Setting, p80). 

The adventurous explorer feat (Dragon 315, p64) gives the following benefit: "When you ready an action in combat, you can choose to take that action at any time, not just when the condition you specify occurs."  That's kind of like an immediate action. 

The mark of Avernus feat (Fiendish Codex II, p84) lets you attack or use a spell-like ability as an immediate action once per encounter.  Unfortunately, it mostly requires being a devil. 

The divine defiance feat (Fiendish Codex II, p83) lets you counterspell as an immediate action by spending a turn undead attempt.  It's way better than reactive counterspell, which has that "takes the place of your next turn" clause.

On the one hand, I did title the thread "Pardon the interruption". 
On the other hand, it's not an immediate action (which is the point).
On a third hand, that just reads pretty-much how I handle held actions; so to me, it just looks like a waste of a power (yeah, I let people break-up their action for free if they want).
Is in the same boat as the Adventurous Explorer feat.

Do readied/held actions warrant mention in this guide? 
I hadn't thought about it before now; and I honestly don't know how to answer that.

The capstone of Anima Mage bears mentioning; the ability to cast any spell as an immediate action should not be overlooked.

The thing that breaks it is when you use metapsi feats (like Twin Power) on it...

There's probably more un-Dazed stuff in a Wilder guide or two.  XPH is where they introduced Immediates, so I bet at least 1 is bad borkt awesome.  Empty Mind 1 is restricted to Will Save but makes a decent Linked metapsi kicker.

Xendrik Lore 7 result is 2 Action Points to make a Standard spell into an Immediate.  That's kinda excellent ... except idk squat about getting 2 Action Points.

I should pour through more of the psionic stuff -- I'm sure there are some buried gems somewhere.
As for action points ..... Eberron is what I know the absolute least about. 
I'll probably start hitting it this weekend (I've hit a wall of writer's block; so I've got not much else to do).

I should have probably been more thorough in my compilation before I posted it.  I just spat out most of it off the top of my head -- figured I'd post it while it was still fairly fresh on my mind.

I was actually driven to write this because of research I was doing for a War Weaver I played a couple of times.

What about using the level 8 Diamond Mind stance (Stance of Alacrity) to get an extra immediate action this turn?  What about using Ruby Knight Vindicator to get extra swift actions?

RKV I don't think warrants mention -- this is specifically "immediate actions", and RKV is but "swift actions".

Ruby Knight Vindicator lets you trade your Standard Action for another Swift action. While this doesn't allow more than one Immediate, it allows both an Immediate and Swift action to follow on.
