The planar landscape is the standard-issue Great Wheel, with a few extra planes here and there(essentially: If you need a published plane to work for your character to function, it does). The ethereal plane, of course, is absolutely covered in demiplanes.

Prime material is basically one big war-torn wasteland. Various city-states are founded and run by mage-lords until they're gobbled up by some other empire or schism when their leader dies. The mountains surrounding this central region are so shitty that no-one wants to conquer them, so naturally monstrous humanoids live here. Whenever someone feels the need to build a dungeon for whatever reason, they go here as well. The gist of things is that the entire continent is just one big playground for nigh-omnipotent mages fighting and backstabbing each other, and the few good mages around generally have a hard time because they lack access to delicious stuff like Dark Craft gold and XP, while not having any significant edge over the evil mages.

Under the surface, in defiance of all laws of physics and geology and logic, lies the grimunderdark. Living here is considered really shitty by surface race standards, so various subterranean races just chill out here. It is dominated by the Illithids, who have grown to such a level of power(due to Illithid Savant being easily accessible) that they have completely stopped giving a fuck. Occasionally they kidnap human commoners to breed, or powerful mages to steal their class features, but no-one really cares because commoners are expendable and a mage with his  brain eaten is one rival less. If they really wanted to they could probably just march out of the underdark and steamroll the surface world, and barring divine intervention, everyone would be pretty fucked. Thankfully, they're way too genre savvy to do this, and they don't actually care enough to consider it worth the risk.

The other underground race with any sort of power is the Phaerimm. Using all sorts of free spells they've managed to survive, although they're still powerless to prevent the Illithids from routinely eating their delicious brains. They're all extremely pissed about not being the underground master race, and will not hesitate to take this out on anyone they can.

The greater doppelgangers have realized that the entire universe is pretty much their bitch, and just spend their time trolling absolutely everybody.

In the forests to the west, the Shaedlings have used shadow gossamer artifacts to completely eradicated any opposition, except a few races they've kept as slaves or cronies. In this case, like with the Illithids, civilization's one saving grace is that the Shaedlings don't actually care.

The Pantheon is the standard DnD pantheon(except restatted to not suck), plus a few extras(your custom deities among them)

Nowhere

Nowhere is a demiplane on the Ethereal plane created by Ransom 10 years ago.  To someone with see invisibility or similar, the landscape is mostly fertile green plane, dotted with lakes and rivers.  A mountain rises in the distance, and the hills at its foot show signs of civilization.  At the exact center of Nowhere is an enormous oak tree, over 100 feet tall.

To anyone else, the plane is... not there.
