Gaming Advice / Trapped in a Pocket Dimension?

3.5 DM here.

Any suggestions for a novel way for my PCs who are trapped in a "void" type pocket dimension, to escape? They are too low level to gain access to plane shift. I want something that will challenge them and cost them dearly. Non-combat scenarios are more than ok.

ETA: More details about the plane. It is slowly draining the constitution of the PCs so the clock is ticking. The plane is mostly populated by undead and constructs because they don't have a con score to lose.

Thanks.  Tell me though how does this apply to wondrous items.  I thought even a fish could use Say a cloak, or ring or necklace.  I thought even dragons could utlize items that were not their size.
My personal opinion is that this in the end is a DM call. I don't like the idea of a fish being able to wear humanoid clothing despite the size issues but that is my own personal opinion.

Shields don’t change size to match the wielder. You can’t use an inappropriately sized shield.

Weapons don’t change size to match the wielder. You can wield an inappropriately sized weapon with a penalty (see Inappropriately Sized Weapons, page 151). Regardless of a weapon’s size, as long as you can hold a weapon you can activate its magical abilities.

So I think it is clear that magic armor, shields, and weapons don't normally resize unless it is a special ability of that particular item. Magical clothing however the language is less precise...

I personally think the above flexibility in magical clothing & jewelry is grounded in the assumption that we are talking about relatively humanoid shaped creatures. How could a fish wear an earring even ignoring size issues, maybe stab it through his fin? But wouldn't the fin be an analog to hand/arm and not an ear? A fish wearing a cloak? Wouldn't you need a neck? Admittedly these are interpretations. I think this is one of the many reasons why D&D needs a DM to adjudicate.

Via the "Shapechange" rules you still need the required bodypart to wear said item "When the change occurs, your equipment, if any, either remains worn or held by the new form (if it is capable of wearing or holding the item), or melds into the new form and becomes nonfunctional" but again how close to a humanoid body part is good enough, is at least partially interpretation, IMHO. 

Here would be a start of some kind of decision list for magical items:
(1) Is the new creature the correct size or can it properly hold the original magic item?
(2) If it is armor, shield, or weapon it does not resize (unless the item specifically says it does). cannot wear it, it melds
(3) If it is clothing or jewelry it will resize or be customizable enough to potentially fit the new creatures form.
(4) If the clothing or jewelry resizes to the new creature's form does the new creature's form have the appropriate body part to wear it? If yes it can wear it; if no it cannot and that item gets melded into new form. 
(*) If the table goes in for Rule 0 then the DM could make house-rules to allow exceptions to the above.

Shapechange: This spell functions like polymorph...

Polymorph: This spell functions like alter self...

Alter Self: When the change occurs, your equipment, if any, either remains worn or held by the new form (if it is capable of wearing or holding the item), or melds into the new form and becomes nonfunctional. When you revert to your true form, any objects previously melded into the new form reappear in the same location on your body they previously occupied and are once again functional. Any new items you wore in the assumed form and can’t wear in your normal form fall off and land at your feet; any that you could wear in either form or carry in a body part common to both forms at the time of reversion are still held in the same way. Any part of the body or piece of equipment that is separated from the whole reverts to its true form.

By the rules above (3.5 SRD) it appears that it is not only armor that can potentially be melded. If you are holding a spear (i.e., a weapon) and you Shapechange into a fish (that does not have arms or the capacity to hold a spear) the spear would be melded into your new form.

ETA: The Shapechange/Polymoprh/Alter Self/etc. spell line and related spells and powers has a long history of changes, errata, confusion, and abuse. See this thread for more rules.

What's it called? I am nothing if not adaptive.

It's for a system called Abandon All Hope which I describe as a space prison in a hell dimension. Also the adventure is basically a choose your own adventure style to bypass character creation and the into adventure called The Program but it would not be easy to adapt.
I'm still curious and at least want to check it out now, can you link it or name it? ETA: "The Program" thanks  :)

Far as I can tell, it was the development title until it officially became 5E.
Why wasn't it always just 5E? Were they equivocating between 4.5E and 5E or something? Seems like "Next" is a way to hedge one's bets ... but what was the bet I wonder?

When Black Roses Bloom for AD&D 2nd Ravenloft.

Like many Ravenloft modules, it is frankly not very good, and makes heavy use of heavy-handed railroading when the players deviate from the authors One True Solution, and wades a lot through Lord Soth's back-story.

What was/is the history with the "Next" naming convention for 5E? Was it preliminary and not official? Was it official and then dropped? Is still in play? What's the scoop?

I'm looking for adventures that have the PCs trapped entirely inside an illusion or dream. Not just an adventure the uses illusions per se but one where the PCs are held within a realm/location that is either entirely or largely not really real. Any suggestions welcome. 

ETA: Would be cool if there were impossible scene changes and the PCs would be granted temporary "powers" applicable to the scene.

Follow Up game-Theory Question: Do you think it is desirable/balanced/sensible that spellcasting is largely safe from being interrupted? I personally don't have a problem with it but my table is crazy worked up about it, thinking it is a radical break from D&D tradition and a sense of verisimilitude.

Note: For some reason the two biggest things that my 5e table (I'm not the DM) disliked about 5e is facing/flanking and opportunity attacks/interrupting spellcasting. Everyone but me is really bothered by these rules. We've largely solved the flanking situation with the optional rules in the DMG but the opportunity attacks/interrupting spellcasting still have them upset.

In 5e, outside of counterspell, can you interrupt a spell being cast that is not a concentration spell?

My group has a problem with a hulking barbarian being right next to spell caster and not being able to interrupt the caster like you could in previous editions even with a ready action.

Q: What are some ways that a recurring enemy can really piss off my PCs (without going too far)? I'm particularly looking for non-combat interactions. 

Campaign Notes: In my particular game I have a sentient evil book that teleports around. Currently the PCs aren't fully aware of the books abilities or intentions and sometimes have even used it themselves.
