I'm looking for a template that would be the closest (at least mechanics-wise) at emulating the transformation into a Hulk (yes, the Marvel Hulk, although not necessarily THE Hulk, so decrease in intelligence is neither necessary nor preferred).
Magic is for weaklings.

Are you asking for:
1. A template which represents the Hulk form of the transformation, where the base creature would acquire the "Hulk" template during transformation and then lose it again when turning back?
2. A template which includes the mechanism for transformation to and from the Hulk?
3. Any mechanism (not limited to a template) which would allow for transformation into a Hulk?

As stated in the OP, I'm mainly interested in the mechanics.
No homebrewing. I know I could just homebrew a Hulk template, and I might do that someday, but only as a last resort.

There's the Goliath barbarian substitution Mountain Rage, which increases the size and strength, etc.  You could add that onto a template perhaps.

The proto-creature template from Bestiary of Krynn gets a rage ability that triggers during encounters much like a badger's rage ability when they get hurt.
