The feat "Faerie Mysteries Initiate" from Dragon Mag. lets you use Int instead of Con for HP, which would help you a lot.

1. Use ranged weapons*, spells, or something similar.
2. Ride a mount. You don't need riding feats or anything, just skill ranks. You might be better off being a small race so you can ride a medium mount and go into dungeons more easily.
3. Have your mount take moves while you do ranged full attacks.
4. Shamelessly kite the ever-living crap out of enemies. Don't open doors, look at things, or talk to people either. Let your meatshields party members do that for you.


* For a damage build, I'd go with Musket Master Gunslinger5/Trench Fighter4 if firearms are an option. If you're in Guns Everywhere, then Musket Master3/Trench Fighter6. This will give you dex to damage twice while swinging against Touch AC. Overall respectable damage output from a distance. Take the standard archery feats, maybe Mounted Combat  too. Deflect Arrows is a possible option because you should almost never be in melee, and maybe Snake Style for the lulz if you run out of good feats. The ability to ignore attacks is highly valuable with such low hp.
