Looking for a Monster

I know I've seen it before, but I just can't seem to remember where from. It's got four arms, and has the ability to cast two spells a round.

Maybe the Spell Weaver from MM2? It has six arms and can cast up to six (level 1) spells simultaneously, though.

Oh for crying out loud. I must have gone through that book too fast. I thought it was in that manual. Didn't realize until I looked at the picture again that it actually had six arms. Thanks.
