I can certainly imagine playing one. I'm not a fan of you must do this every day for a whole previous level requirements just like I don't like heavy useless feat taxes - it disadvantages people who actually play through those levels.

Fair enough.  However, if you're building the kind of char that would want horned juggernaut, you'd want to use RR in most fights.

Quite cool but I was expecting the ability to Open/Close Gates (as the spell) though admittedly the capstone does something along those lines.

Finally getting details hammered away, and have decided on my base spell. Definitely will need some refining of the language when I'm done posting it all. :D

I don't think I'll be pursuing my current entry's direction. It just isn't grabbing me, and feels a bit bland. I may or may not be back to finish it in the future (and/or make an alternate entry), but I'm not holding my breath.

On the Open/Close PrC, should probably be noted what effect opening someone's eyes has on gaze attacks.

Would also be interesting if you could create temporary empty spaces passing through objects, replicating passwall, parting water or area effects, creating a safe path through difficult terrain, etc.

Juggernaut Charge is an epic scope ability (see the similar ability of the Legendary Dreadnought) and, between the per encounter su and the character's spell slots they are not in too much danger of running out of uses. OTOH it would be pretty cool to have the transformation last longer. Perhaps it should last con bonus rounds but only so long as the Juggernaut keeps moving i.e. it ends if the Juggernaut finishes his go without having moved at least 10' in the round.

It's not just the uses, it's gaining the benefit of some of that stuff when you're not charging, and when it isn't your turn.  At the moment that Con & natural armor only help you with AoOs and readied actions...
