So, casting wish to make, say, a +10 sword would cost 21,000 xp. The normal price for that wish spell is 106,530 gp. (As an aside, has anyone noticed that before?) A sapphire hierarch can get it for 10,653 gp.

No... that's a bit nuts, isn't it?doesn't it say somewhere that when an xp cost is involved, npc casters charge at least 1gp per xp or more, and many refuse to do so if there is an xp cost? or was that already included in that cost calculation?
The larger issue is securing a 19th level caster is difficult, as it is not guaranteed to find one.

Per the rules, you can buy a +10 weapon for half price by paying someone to Wish for it. That means there is something really wrong with the rules...

See, by default metropolises do not have casters capable of casting 9s.Or, in other words, your DM has to be nice for you to be able to purchase 9th level spells.
You can't go to Sigil because the 10% price spells are only at the temple of the Sapphire Eidolon. Their singular temple which is, by the description, in the middle of a remote wasteland. So no going to Sigil and finding the best spellcaster money can buy, no looking up the typical highest-level spells of a metropolis, none of that.
Ah, I misread a bit. So the issue is that it is cheaper to have a wizard Wish a sword for you than it is to have him make it?
Where does it say again that the city of Sigil has sucker 17th level casters that will happilly answer your summons?
no gp limit applies when buying or selling goods in the City of Doors, and NPCs of any class and level combination can be found there.
