A true Lock-Down has the following abilities...

I once read someone write that a true lockdown has the following:

AoO for being attacked
AoO for enemy movement, even a 5ft step
AoO for enemy magic, no defensive casting
AoO for enemy magic item use
AoO for enemy doing nothing
Dual-wield Kusari-gama, take double-hit feat. Double your AoO's
Stop enemy from being able to do anything with AoO (Stand still for movement of big things, tripping for small things, etc.)

I'm wondering how to get each of these?
AoO for being attacked ---- ???
AoO for enemy movement, even a 5ft step --- Thicket of Blades + Best Reach you can
AoO for enemy magic, no defensive casting --- Mage Slayer?
AoO for enemy magic item use --- ???
AoO for enemy doing nothing --- ???
Dual-wield Kusari-gama, take double-hit feat. Double your AoO's --- ???
Stop enemy from being able to do anything with AoO --- Stand Still + Improved Trip.

I'm not sure the "AoO for doing nothing" exists...

Of course it exists. It's called "Full Attack on your turn unopposed"

AoO for doing nothing is Defensive Sweep.
AoO for being attacked is Robilar's Gambit.
AoO for being hit (not on the list) is Karmic Strike.
I'm not arguing, I'm explaining why I'm right.

Supernatural Opportunist might also be useful, depending on the game.
I don't pee messages into the snow often , but when I do , it's in Cyrillic with Fake Viagra.  Stay frosty my friends.

That one stance for an AoO for any movement.

Drunken Master can improvised weapon a pair of rope to Double Hit at 50ft

I'm wondering how to get each of these?

Not sure why AoO for being attacked falls under Lock Down.  It's helpful sure, but...kinda a different animal.  What SHOULD be on that list is "AoO for attacking [not me]."  To which I will add the Backstab feat from dragon mag.  Against an enemy you flank, each attack he makes that isn't against you provokes an AoO.  Combine with Island of Blades or Vexing/Adaptable Flanker for best results.

Lovely, thanks for all the help guys! Also, Defensive Rebuke is a 3rd level Devoted Spirit boost that causes enemies who attack others to provoke AoO's.

Might be a bit too niche, but I always loved Cometary Collision + Steadfast Boots to protect the party from charges, too.  Adding in Pounce (to make your counter-charge even MORE painful), Knockback (so they end up with nothing.....NOTHING!!!! for all their efforts and suffering, muwahahahaha!), and maybe even Hold the Line (so they get whacked even more for daring to charge) is just so awesome.  Cometary Collision has a protection radius of your charging range and triggers if the enemy passes through that radius at any point in his charge, and halts it with 100% accuracy.

It is one of the very best ways to utterly curbstomp the entire enemy's use of charging.  Especially if you play Steadfast Boots by RAW and you can keep gaining "readied" actions to set vs. charge each time someone charges.

Slightly more efficient, Spear of Doom + Steadfast Boots. Though it does nothing for protecting party members.

This is awesome. I'd been toying around with combining Cometary Collision + Steadfast Boots, but hadn't dug around to find all the good things to add onto it. :D

Crowded Charge, Joint Bull Rush, Massed Charge; for the Scrum! of it.

Huh, would that work against an NPC version of Endarire's Hood ?!

It'd be slightly to significantly low gear, and have some trouble
with the skill checks to determine why the Shtick hit the fan.

Hmm... looking at the feat's wording, I'm not sure this even works together... The boots basically treat you as being "set vs. charge", while the feat requires its own standard action to ready to use.

It's still a potentially awesome feat in the right campaign, and of course the boots are awesome in their own right.

And do you have a way to make that count as an AoO?

But defensive sweep. Wow BAB +15 is a massive prereq. It requires shenanigans or it puts you on par with 8th level spells by ECL for regular full casters.

Yeah, Defensive Sweep is quite the pain for the little gain, though it could be used with something like Stand Still and Thicket of Blades to ensure the target doesn't move.  Lowering its BAB req to 12 or 9ish would make it more useful for sure.

Something fun with the Defensive Rebuke maneuver: Thanks to errata, the War Hulk's Mighty Swing ability is an attack action, not a standard action.  It might even work on AoOs.  Or just activate the Whirling weapon enhancement, but that's a limited number of times per day.

Alas, if it weren't for the No Time to Think class feature then War Hulk and Tactical Soldier could be a nice combo.

Shame all these can't be combined on a single character before Epic levels, unless it's gestalt perhaps.

Not only COULD you....that's the only reason any one EVER takes it at all, IMO.  I sure as hell would never take it w/o some means to ensure them 5 ft stepping *also* provokes.  Because on its own it really truly is a horrible super-niche feat that will basically never come up and costs one of your extremely high level feats.  It's only in tandem w/ Thicket of Blades that it becomes useful at all.  Considering ToB came out after PHB2, I presume the authors never intended for it to actually be useful and it turning out so was a wonderful "mistake."
