Is there a race or template that will grant polymorph as a SLA?

When 3.5 errata'ed polymorph, they pretty much systemically removed it as a SLA from nearly every creature that had it.

The only one that springs to mind (and there might be others) is one of the Dragonlance books has an alternate ghost template that can choose polymorph at will (on itself only) as a SLA.  So do you want telekinesis, or polymorph?

Technically speaking, I suppose the Chosen of Mystra and Magister templates from Forgotten Realms would let you pick polymorph as your 4th level SLA.

Dragonlance:  Age of Mortals -> Bound Spirit template.  Makes you incorporeal undead, with polymorph as a freakin' (Ex) ability at will once you reach 12 HD.  It is +4 LA.
